Download any of these convenient forms, and

upload them to your Patient Notebook account. 

 

Instructions for uploading attachments via Patient Notebook:

 

1. Go to Patient Notebook via this link.

2. Log into your account. (First-time users, check your email for your invitation code provided by Patient Notebook.)

3. To upload your completed forms, select "Messages" from the top menu.

4. Click the box marked "New Message."

5. Select "Maternal-Fetal Medicine of Maryland" from the drop-down menu.

6. Choose "New Patient Paperwork" from the second drop-down menu.

7. Click the paperclip icon next to the word "Attach" and select the forms you'd like to send us. (You can attach multiple forms.)

8. Hit "Send" and wait for confirmation that your forms were sent.
