If
you have Carefirst BlueChoice HMO insurance (not open access)
you will need to provide our office, upon your arrival, with an official
BlueChoice referral from your PCP (Primary Care Physician who is listed
on your insurance card) They may
fax it to us: 301-315-2169 or you can pick it up at their office and
bring it to your appointment at our office.
- The
referral may include up to 3 visits.
- The referral should say high risk pregnancy
and/or Consult and Treat, or the actual reason for being seen at our office
(Not
global OB care).
- A referral from your OB will not allow a Carefirst BlueChoice
HMO member to be seen in our office, it must be an official referral from
your Primary Care Physician listed on your card.
- If
you should arrive to our office without an official referral from your
PCP, we will have to reschedule your appointment.

- What do I need to bring to my first
appointment?
Please bring the following to your first appointment:
- New patient packet completed other than signatures.

- Valid Insurance Card
- Photo ID
- Insurance Copay for Specialists Office if applicable (i.e. on card...OV -
$15 or Specialist - $15)
- Any pertinent Referrals based on Insurance Policy
- Any pertinent lab work necessary for physician
